Please review the information below to assist you in completing the online KAMCAS referral form

Referrals will only be accepted if completed by a social worker or health professional working for or on behalf of the following secondary services:
• Adult Social Care (Kent or Medway)
• Community Mental Health Team
• Eating Disorder Service
• Personality Disorder Service

Your client must meet the KAMCAS referral criteria:
• Individuals who have a primary diagnosis of autism (without a co-occurring intellectual disability).
• Individuals for whom local services and health provision have been inadequate to meet their needs.
• Individuals who are considered ready for discharge from hospital or release from prison, or who are unable to retain their place in the community due to significant risk in the medium term (9 months) of: hospitalisation, conviction, court disposal, suicide, and/or severe harm to self or others.

You will need proof of your client’s autism diagnosis, and will be asked to upload evidence of the diagnosis, preferably a diagnostic report. If you are not able to provide evidence you will not be able to complete the referral form.

You will need to confirm that your client has consented to the referral, alternatively you can provide evidence of the Best Interest Decision.

Throughout the form you will be asked to upload documents such as evidence of co-morbidities, care and support plan, care needs assessments etc.

You will be able to save your form entry progress before final submission to KAMCAS.

Please note: Uploaded files will not be saved and will have to be re-uploaded before final form submission.
